Primary Driver Information

Insurance & Authority

Please provide copies of current Insurance Certificates (ACORD) showing Brahmos Logistics LLC as the BROKER and as a certificate holder and additional insured with the following minimum limits unless otherwise agreed in writing: Auto Liability: $1,000,000, General Liability: $1,000,000, Cargo Insurance: $100,000. Insurance must remain in force during the term of any haul.

Payment & Factoring Options

Select your preferred payment method and provide required details. If factoring, include factoring company name and a signed factoring agreement or notice of assignment (NOA). For direct deposit (ACH), include a voided check or bank letter.
